Nursing Revalidation – forms and templates from the NMC

If you are starting to complete the new nursing revalidation portfolio, you can download the NMC’s mandatory and recommended forms and templates. Forms include:

  • Practice hours log
  • CPD log
  • Reflective Accounts form
  • Reflective discussion form (with guidance sheet)
There are also some examples of completed forms for you to have a look at.
